In my first year at university I got a 3.28 GPA.
In my second year, i was diagnosed with depression and began to start medication. since then, my GPA increased to 3.67 in my second year, 3.8 in my third, and (hopefully!) a 4.0 in my fourth (which I'm currently in).
Should I submit an addendum to account for my grades in first year?

Depends on what your final GPA is. As much as some adcomms, and I, certainly empathize with the situation, law school can be really stressful and a lot of adcomms would wonder if you could thrive in that environment. If your final GPA is solid I wouldn't take the chance, if you end up with pretty rough overall GPA I prob would so as to draw attention to the upward trend with treatment.

I wouldn't. It's pretty common to have your freshman year be your worst (3.28 isn't even that bad) and there's unfortunately a stigma with depression that is probably better avoided than unnecessarily addressed.
