I just looked at LSN and that's not true. Like 90% of people at or above both medians were accepted. There was something off putting in their file. Numbers are a big determinant, but being above medians in no way guarantees acceptance. You are talking about Columbia.

Well, a number of things can prevent admission at any given school.
Display of lack of maturity in any facet of the application, including one's age or lack of work/life experience
An extensive criminal record
A bad interview
Sloppy essays, including topic choice
Bad recommendations
Writing, "Fuck you because I know you don't read the LSAT writing sample," on your writing sample
Over zealous/inappropriate communication with the Admissions Office
A display of a sense of entitlement to admission
Basically, think of any way an applicant can be off putting, and that could be why those people were wait listed or denied. Also, add in the fact that an applicant can be wait listed for simply not meshing with a file reader on that particular day. It really is the job of an applicant to impress them AND make them like you.
