Character and Fitness
So I'm applying to t14s as a reverse splitter (168/3.9) and I have a little thing on my undergrad disciplinary record (I was in possession of alcohol underage, but not drinking, I was holding it for someone else). My school, believing this, decided not to pursue disciplinary action, but still left a note in my file that it happened so if it happened again they could point to that.
First, do I need to report this? I'm assuming yes.
Second, if yes, will this hurt my chances of admittance seriously?

Definitely disclose. Also retake if you can.

Just want to echo the retake sentiment in case you missed it. Improving just a couple of points will be worth tens of thousands of dollars in scholarship money, open up a great range of schools (that also won't care about your minor C&F issue - but disclose it), and be worth un-quantifiable amounts in future job potential/earnings. RETAKE!

i'll echo what everyone else is saying
but especially what people are saying about retaking
it's a no brainer
eke out a few more measly points on the lsat and you'll save yourself tens of thousands of dollars (maybe hundreds)
it's worth a couple months of studying (unless you're obscenely wealthy)
