It says to "explain fully". Does this just mean detail the infractions and what immediately caused them? As in, "I was placed on academic warning because I got a D...", etc., or does it require a further explanation, as in "I was inexcusably apathetic that term...", etc.

There's no standard but typically you want to be as brief as possible while accepting responsibility without making excuses. So for you it would be something like "During the fall semester of 2009 I was put on academic suspension pursuant to school policy mandating this should one's GPA fall below X. In subsequent semesters I took greater responsibility for my academic performance and my grades significantly improved"
