Yale activities and work

how did people format this-- literally just list things, is there suppose to be any explanation of things? What is the point of this if you have a resume?

Categorize a, b, and c - then just bullets underneath. Used the résumé for an explanation of those important activities. With columns for hours, etc. otherwise I tried to make them self-explanatory-ish

Rumor is that one of the faculty didn't like reading resumes, so they requested this form with the only things they are actually interested in from what is normally put on the resume.

Should unpaid internships go under self-support (employment) or other activities.
