Carefully read the C&F section of your applications. The questions tend to ask for specific information. If you have further questions, contact the admissions office(s).

I would go for full disclosure. If they somehow find out about it in the future, it'll look a lot worse that you didn't mention it to them. Most schools say that you still have an obligation to update them if something happens after you submit your app.

Verbally warn, but not written up? If so, you are fine, I don't remember verbal warnings being an issue on any of my apps. If you are ever so dreading being disbarred for it, just disclose. No one will care anywhere.
