Call the school & see if they have an agreement with LSAC. I had this issue with DePaul. They will not attach anything to transcripts, but they have an agreement w/ LSAC. My transcript was sent & processed through LSAC.

People do send in transcripts without the form with the barcode. Most seem to eventually get put in the right file, although if you have a particularly common name there could be issues. The barcode is really just to make the process a little more efficient and less error-prone.

I sent in one of my transcripts without the form and it made it in perfectly fine.
