I don't plan to apply until probably fall 2013, but I am going to leave this job soon and my supervisor has agreed to write a letter of recommendation for me.
I understand that they also need to attach and fill out some kind of form from CAS in addition to the letter?
How does the process exactly work?
Does the recommender need to send it the school directly, as opposed to me receiving a sealed envelope and sending it out?
Do I need to give him a form and ask him to put in the sealed envelope with the LoR?
Answer may already be on the forum somewhere....couldn't find the exact answer I was looking for

print out the form
they mail the LOR with the form directly to lsac (address is on the form)

You list the person as a recommender on LSAC, and the site generates a PDF form for you to print out or email to the person, for them to include with their letter. They mail it directly to LSAC.

thank you for the help. Just one more question (for now): since I am planning to apply next year, what would happen if my recommender finishes his recommendation and sends in...say next week?
Will LSAC keep in their records and then send out during the next admissions cycle or do I need to wait until I ask for the recommendation?

LSAC will keep the the LoRs for as long as the account is active (5 years if I'm not mistaken), and you get to choose when to send out the recommendation. So the sooner your supervisor sends in the recommend, the better. (The recs I used for my application were more than a year old).
