I attended School A from August 2008 - November 2009 when I took a medical withdrawal. I attended School B from January 2010 - May 2011 and did receive my Associate's Degree at that time. I'm currently in the process of attending School C (since August 2011) where I expect to receive my Bachelor's Degree in December 2012. Do I list all three schools on the resume I submit with my application? Do I only list the two where I earned degrees? Do I only list the school where I will receive my Bachelor's? Any opinions would be welcome!
Also, my grades for the Fall 2009 semester are all "W"s due to the medical withdrawal. Obviously, any law school will see this on my LSAC report and have questions. Would it be a good idea to submit an addendum and explain the medical withdrawal? It was for mental health issues (severe depression) if that matters. Are addendums submitted to each school separately along with the individual applications? I believe that's how the personal statement and resume are handled. If not, are they submitted through the central LSAC system? Thanks for any help/advice!

On the resume only list the degree granting school.
On your law school app you will be asked if you've ever taken a break in school. When you answer yes you will have an opportunity to attach an explanation.
On your app you will also have to list all of the schools you've ever attended for college including the schools you transferred from.
