I just downloaded the one from Stanford because according to this website (which I've heard is a bit outdated but works nonetheless) --LinkRemoved-- S is the only T14 that requires it.
I called Stanford and they told me that the Dean's Cert would be the same so it was safe to go ahead and get that one filled out. The form, which can be found here
http://www.law.stanford.edu/program/deg ... _formb.pdf has an address at the bottom and the Dean of Students Office will take care of sending it directly to that address.
If you are doing the Dean's Cert for Stanford they told me on the phone that I should indicate on the form that I'm an applicant for the Fall of 2013 since I'm submitting it earlier than normal (the Dean at my school was really eager to get it in before the mad dash of applicants come fall).
The only other school I see in the top 30 requiring a Dean's Cert is U Washington, haven't checked that out but I'm sure it's on their website.
