You can get the index on the LSAC site. Log in, go to Apply > Credentials > Transcripts, and there's a link to the PDF near the bottom right.

According to the index, Colorado, Seattle and Vermont are the only schools that use degree GPA and not cumulative.

(Alternatively, you could quit freaking the fuck out and study for the LSAT.)
