Find out who actually granted the credits - mine came from a local community college even though I took the exam at a public university. I needed to request the transcript from the community college even though it showed only six hours of pass/fail credit.

They didn't ask me where I wanted them sent. They always showed up as transfer credit. I would send the transcipt to LSAC. They'll let you know if you need to send anything else.

Took the exam at my undergrad and had the scores sent there. The credits showed up on my undergrad transcript as transfer credits. Nothing else needed to be sent.
