C & F Format

Is there any kind of format (salutation, etc.) that I should use on a C & S addendum?

Do whatever you want, as long as it's formal.

I did the classic: Dear Sir or Madam

Sorry, I should have been a little more specific. Should I include a header and all that jazz, or should I just have a formal salutation?

On a similar topic, should there be any format to the PS?
