Stanford App? Statement of Undergrad Dean?

Has anyone done the Stanford app? What is this statement of undergrad dean? Is it required?

Yes I've done it. Stanford is one of the few schools that requires it as a part of your application. Many schools require it upon matriculation. Contact your school's Dean of Students (or equivalent official) to fill it out and mail it to Stanford. The form which you can get online from Stanford (and it may even be available on LSAC) just asks them to fill out a few questions concerning your academic and disciplinary conduct.
