T14 schools that want an LSAT addendum for big score jump

Specifically regarding the 2011-2012 cycle.

Hoping we can compile collective knowledge here.

What schools and in what situations?

Can anyone relay info from adcomms?

From Michigan's site:
The average score becomes less useful as the disparity between two scores increases; for that reason, if you have a significant disparity between scores (six or more points), it would be very helpful to address any explanation for the difference in an optional essay or addendum.

Chicago has asked for one in the past, not sure if they still do.
