Did LSAT get my LOR? (Semi weird situation)

Ok, so I am having 2 LOR's. One of which last Friday I went into his office / gave him envelope and he put LOR into it / I saw him put it into mail... The other of which said he'd write it for me, I had some difficulties reaching him but then on last Thursday he saw me in library while I was taking PT and came over and said that he "had me covered on the LOR" and to come up to his office in 5-10 (his office is in the library). I figured that meant it was complete and to come to come up to finalize on where to send it and all that good stuff (I had my signed letterhead with me). I only had like 10 minutes left on my timer for that section, so I finished that section and then headed up to his office and to my dismay he wasn't there (he seems to NEVER be in his office). I waited for awhile, then kept stopping by and he was never back.

Today I got e-mail from LSAC

The only problem is the name in the subject is the one I never found to give him the last stuff on the LOR / not the guy I saw mailed it. I never even gave this guy the signed item from me that LSAC requires when they mail in LOR. I've been trying again to reach this guy this week but its become frustrating.
Which is more likely; him being a professor knew where to send my LOR / just sent it in himself after telling me he completed it OR LSAC made a mistake on the LOR they really received and actually got my other one? Does it matter that it did not have my signed cover letter?

Nevermind, I call LSAC... they made a mistake it was under the wrong the name
