evaluation v LOR
I'm 2 years out of undergrad and will have two letters for my apps--one from my boss (I've worked here fulltime since graduating) and one from a prof I had for two years. My question is this: would it be okay to have my supervisor fill out both the evaluation and write a letter? Or should she only do the evaluation? What did you do?

You can have one person do a recommendation and an evaluation.

I don't know? But I saw the questions and I think they'd be useful for my supervisor to fill out, so I thought I'd throw it in there. It can't hurt, right?

Not unless you get a bad one I suppose. They were totally optional and everyone completely ignored them last cycle.

Does it make sense to submit a recommendation and evaluation from the same professor to the same school?

I would ignore evaluations. Northwestern was the only school who tried implementing them and it lasted two years. Some schools even discourage them.
