Character and Fitness

About three years ago I received a formal reprimand from my school for violating my schools student conduct code (underage consumption of alcohol). I had to take an alcohol awareness class and I never had another incident. It was never a police matter but a school matter. Do i still have to disclose it on applications, and if so, what should i write in an addendum?

Yes, you should disclose it. Just write a brief (three or four sentences) addendum explaining that you took responsibility for your actions and have not had any problems since. It's going to look much worse if you try and cover it up and then they find out about it later.
