Dean Certifications- Character and Fitness

Some of the schools require those that answer yes to a couple of the character and fitness questions to submit a letter or certification from their dean. Most of these schools give you a form that you deliver to your dean. However, Duke says to just have your dean write a letter describing the incident and to send it in. How do I go about doing this. When I checked my schools site it said to drop off dean certiifcation letters with envelopes and stamps at the office. Well, since Duke has no official certification form what should I do?

Obviously call them and ask. How would we know what your school wants you to do?
