They won't care where you went, all that matters is your ldsas gpa. Dont sweat the small stuff

dont do it. there is no reason to explain your college choices. only write an addendum for a negative incident in school, a bad gpa ( if you have a legit reason for it) and a large gap in your LSAT scores.

If you really want to you can write an addendum on why you wrote an addendum. That may go over well.

No way. Even if it was University of Phoenix or DeVry (which I'm assuming it wasn't since you were able to transfer)

Don't write an addendum. But these sorts of things can be good to work into your personal statement so long as it doesn't come off as an "I'm better than my crappy school" kind of elitism.
