send an e-mail. i don't think anyone will really be really annoyed by an e-mail and just trash your application or LOR

hmmm... I wouldn't draw attention to it... 1) it's not a big deal, and 2) the adcomms won't know whether you or your employer made the mistake.

I wouldn't do anything. They're not sitting there with your resume and letters cross-checking minutia.
