How do you end an addendum?
Do you end an addendum with "Sincerely" , "Best Regards" , "Thank You" , or nothing at all ?

I really did mean that "nothing at all." that's what I did on mine. Just a short paragraph describing your particular situation, not a whole letter-formatted thing.

During the October 2010 LSAT my irritable bowel syndrome flared up. I was in intense pain, but I decided to continue with the exam. In hindsight, I should have run to the bathroom and taken the misconduct warning from LSAC. Too little too late was the phrase of that day. However, I would like to inform your school that my December 2010 LSAT score is truly indicative of my potential. I urge the committee on admissions to place as much weight as is possible on my higher score. Thank you.

After addressing the matter with LSAC, I have been told that I will be allowed to retake the test once more. I didn't put anything at the end of my addendum. Regarding spacing, I would use the same spacing/font/font size/header that you used on your personal statement and anything else you attached with your application just to make the whole thing look more organized and uniform.
