To Address or Not Address

Should traffic ticket addendums be addressed to the admission committee and indented formally. If not, can one just title as "Addendum regarding Question 26" and not indent or address the admission committee. By address I mean "Dear selection committee". Secondly, if I wrote "I pled guilty to this violation and once again paid the fine promptly". Should there be a comma after the word again. Thanks.

I didn't address any of my addenda and used headers similar to the ones you are suggesting (but more generic). FWIW I question whether the adcomms have memorized what Q26 is.

Naw they probably have, since it probably says on the application, "If you marked "yes" in response to question 26, please include a comprehensive addendum explaining your blatant and wanton criminality."
