I took the LSAT twice, and improved 9 points on the second take. The first time around I was quite ill, had to use the bathroom mid test, and was generally so incredibly nervous that I psyched myself out. 
However, I also spent a lot of time focusing on my weakest section before the second take, and that helped me to improve my score as well. Should I list both of these issues on my addendum, or just the illness?

How much detail should I be going into about my illness? I don't want to get into the gruesome details, but I also don't want them wondering what the hell "I was feeling ill" refers to and wondering whether I am making up stories.

Just say that unfortunately you were ill during the test.
I gave a couple more specifics, saying that I was ill and unable to sleep the night before.
