NU Career Progress Evaluation Form

Want to apply NU, but it says that the recommendation must go with CPE Form?
What is this form actually? My recommenders have already sent their LORs to LSAC, do I have to ask them fill the form??
Anyone has good ideas?

The instructions are a little misleading on the application. The CPE is in lieu of LORs and neither are required. However, it seems they prefer the CPE.
