I don't know any more than you do but my opinions are ...

1) It probably doesn't matter whether it's on one page or two, I would probably do two.

2) Not sure about this

3) A lot of people write addendums when they have an upward grade trade. If you became a much more serious student in the last few years that is worth mentioning I think.

I don't think a school that accepts a 155 will require an LSAT addendum. They will probably just take the higher score.
