Addendum for Disciplinary Action in College Forum

I've written an addendum for disciplinary action in college. This was done to the best of my abilities from recollection, as there is no information at the school regarding the offense (it was expunged upon my graduating).
As it was 8 years ago, I'm not 100% sure if it's accurate, though I think it is (I called up past roommates to verify since they had the same offense as me). How should I bring this up? Just a paragraph at the end stating the university policy so I was going off memory of 8 years ago?
