LOR request form + UGPA Q Forum

Hi, two potentially stupid questions...
I was wondering...I'm not anywhere near the professors writing me my LORs...how do I get them the form? Just fax it to them? I don't understand why they don't make it a PDF so that I could just email it to them to print out themselves! rawr.

Also, is grad school GPA calculated in the UGPA? I see that it just says 'see trans' in the LSAC summary. If not, and I have Bs, should I still explain them (would definitely if they counted toward UGPA)?
