Harvard Fee Waiver Question

Has anyone applied for the HLS fee waiver via application form? It talks about choosing electronic submission option ---> pay by check ---> and mailing in the fee waiver app in with the "certificate". What is this certificate, and is this something HLS provides or LSAC at the check out?
