Include name in Personal Statement/supplemental essays?

Hi guys,

I'm wondering if I should include my identifying information on my PS/DS (ie put my name/SSN at the top)? Or is it fine to just attach it to the application? Thanks for your feedback!

I would always include my name on the personal statement, and only include the ssn or LSAC account number if the school explicitly states it. Even though it is transmitted electronically, it could get mixed up if it is printed with multiple applications.
