LSAC number on your ps/ds/addendum

Sorry I suck at forum searching - are you supposed to put your name and LSAC number on each page of your PS/DS and/or Addendum and resume?
Or just submit it as is

Some schools ask you to. I put it on all my essays just in case. I left my resume as is because it already has my name.

I'm sure you'll be fine on your resume and addendum, since they typically dont go over a page...

I only recall one school I applied to specifically asking for this. Other than that, LSAC sends an electronic copy of your app that has your statements all as part of it, like one running document. Therefore, I don't think its would be an issue and I am not sure why some schools would want your name on each statement.

I just made a header with name, LSAC#, and title of page (like PS or C&F addendum or whatever). I didn't use it on my resume, because i just added by LSAC# right under my name on that page.
