GPA (and other) Addenda

Do I need to provide an addendum if I am missing a semester on my transcript? (Accident + delayed surgeries resulted in a medical leave).

Also, I am planning to attach a brief GPA addenda to put my not so impressive <3.4 UGPA in better context (grade deflating major, graduated in top third, etc). My academic LOR writer told me he had made mention of the relative rigor of the curriculum, but I have no idea whether his letter clarified the idea that "I don't suck as much as my GPA would suggest". With regards to this:
1. Bad idea to include such an addendum?
2. Would I make these two separate addenda if I included both? Or should it be one giant "Academic Explanations" addendum. I am leaning towards the former, of course, but I don't want to be one of those applicants with 200 additional files (I'm already submitting a DS and a WhyX for most of my schools).

Difficult major isn't a worthwhile addendum topic. Your transcript and resume say what your major is. They know whether it's tough or not.
