Who does my recommender address his letter to? Forum

Who does my recommender address his letter to?
I'm applying to 5-10 schools, I've completed the letter of recommendation form for my recommender through LSAC - who does he address his letter to? Does he do one for each school? Help!

or
"To whom it may concern"
something along those lines would work for multiple school I think

They all assume you're applying to multiple schools. And is it true that some schools have a form for the LOR writers to write their letter on? I was just going to have my attorney-boss write it on his letter head.
