I had this problem, too. After reading a couple books and a few hundred posts here, this is what I've done:

As for the profs, just ask them now if they're willing to write you a LOR. Tell them a few general points/anecdotes you'd like for them to touch on, and tell them you'll have some more specific points to talk about when you've done your PS. Let them know you're planning to apply on X date, and ask them if they could have an LOR ready 1 or 2 weeks prior, or even a month if that's still far enough away.

The personal statement can be tricky; everyone's used to having specific prompts for writing assignments, and severe punishments if the assignment doesn't address the prompt. You have to get over that built-in anxiety. What helped me was to read. Law School Confidential, How to Get Into Law School, JustDude's guide in these forums, and the PS advice article on the main site here all helped me. Remember that you just need to write two double-spaced pages on why you should be admitted to X. If you keep this in mind, read the above materials, and follow their advice, you'll be fine. 1) Your personal statement is completely separate from the actual application. You just upload a Word document and it converts to a PDF that is sent along with the application. There is no template for PS's.

2) There is a specific form, but you only need the name, title, phone number, and address of the recommender. They don't write the rec on the form. And again, that is separate from the application. You can have them write the recs and send them in with the form and it will be uploaded to your LSAC account.
