How to denote time off from school on applications?

Basically I went to school for 2 years, didn't go for 2 years, and then went back and finished in 3 years. Most applications only have space for one particular range of continuous dates, but that isn't my situation so I'm unsure what options I have.

You could write an addendum explaining your situation.

I have an addendum explaining my gpa that resulted from that time off but I was more interested in what to put on applications. Should I just include the date I graduated?
