Appeal Letter

I am writing an appeal for a waiver for the lsat. Can it just begin: To whom this may concern or is there a more standard way? Also, how formal is the appeal letter supposed to be?

Are you writing an appear of a fee waiver denial, or trying to get a waiver of that "can't take it more than X times in Y years" rule?
