I would write an addendum on it. In fact, there will probably be a question on most apps' character and fitness sections asking if you've ever interrupted your education for more thn six months or some such.

You don't need to disclose the nature of your illness specifically, but you don't want to leave it to an AdComm's imagination why you withdrew for a semester.
