Addendum formalities - paper, signature, etc.

I'm wondering - should it be printed on high-quality paper at Staples or wherever? Should I sign it? Should I send it in a full-size envelope (ie don't fold it at all).

I am probably wasting my time, but if I send something I want it to be presentable.

I thought addendums were to be electronically attached.

even if u sent a paper application, see if they'll take an email attachment as an addendum.
