My question is, how can you get a 3.9 but then not put in the time and effort to get a good LSAT score?

Well GPA can be a factor of hard work alone, with a moderate amount of intellect, but given the time issue, the LSAT factors in processing speed and mental skills much more. So if Vinny was a really hard worker and always stayed focused, he could have done really well in school, but without other skills he may not have had success on the LSAT. When you have 4 months to get the grades, it's pretty much impossible not to succeed if you TRULY put your mind to it. Most people aren't willing to do nothing but schoolwork, though.
I think that most people can improve their score significantly through practice, but I don't think you can take a 145 and make them into a 180 just by practice alone.

That really depends on what material you are studying. +12 here, coulda improved more but I didn't study particularly well. It was more repeated practicing than studying.
