Titles for PS'?

Should you put anything in the title of your personal statement, or on an addendum? For example, would it make sense to at least put your name and then "Personal Statement" to help avoid any confusion with other applicants?

Don't put an actual title line or anything - you shouldn't have any space for that when you're done writing.
