Is it ok to include the header information "name / lsac # / and document title" in the header of the essay instead of the body (in order to save space)?

that's what I did. It didnt cause any problems.

Doing this won't hurt anything and some schools ask for it, so just do it for all of them.

yeah header is the way to go; use the same header for any addenda
