Or

One neighbor, who my mother fearfully referred to as…

It's a direct object. Whom.

The correct phrasing is awkward here. Why not, "My mother fearfully referred to one neighbor as _________________" and then a new sentence describing whatever it is you were going to say about this neighbor.
