What info should I put in the header for the PS and DS?

This is what I am planning to put on header of my essays :

Full legal name as it appears on my LSAC account
LSAC Account Number
Essay Type (Personal Statement, Diversity Statement, Optional essay #1, Addendum, etc)

Should I also include my social security number in the header as well?

I think what you have is fine. My header says exactly what yours does:

Last name, First Middle
LSAC #
Personal Statement (or whatever the document is)
