I'm a little confused by your GPA addendum. You went on a drug regimen that made you do badly in classes for one reason, then voluntarily came off the medication and did badly because you took yourself off meds? I think you could maybe be a bit more clear by just stating that you were diagnosed with ADHD and the entire process of determining the correct dosage of medication led to poor performance that is not indicative of your true abilities. Considering this was your senior year, you should also give some evidence that this problem is now corrected so the person reading it doesn't believe this will continue to be an issue as you start classes again.

What would be best is if you could give actual evidence beyond, "but now I'm fixed, I promise!" regardless of the phrasing. You've been out for a few years, so maybe evidencing your performance in the real world or some sort of academic endeavor you've had since UG. You might have to get a little creative, but if you've undertaken any task that would've normally been impeded by your ADHD/Adderol use successfully, it could be successful evidence.
