Formatting Questions

Where can I find a list of universal formatting conventions? I'm planning on making my PS two double spaced pages and using 12 pt Times New Roman.

Should I indent? Do I include any additional information (name, etc.)?

Sorry for the silly questions but I didn't really see this anywhere.
