Going by my gut instinct, I'd cut it like this:
As addressed previously in my application, my first year at [College] I withdrew from courses and moved home for financial reasons. Throughout my academic career at [College] I worked a minimum of 30 hours per week, and at times up to 60 hours per week. I understand and take responsibility for the detrimental effect this had on my LSDAS GPA. This experience showed me where my limits lie, and since qualifying for financial aid, my grades have seen a continuous and marked increase in the face of dramatic changes in my personal life.
Addressing the issue in the PS rather than an addendum might be better.
