Hey folks,
After browsing relevant forums, I've decided not to title my PS. My question is whether I should including ANYTHING at the top, including page numbers. It looks rather bare at the top with absolutely nothing.

I put numbers too. Like name/LSAC # on the left margin and page numbers on right margin. Can't hurt to have them and might just make something easier for an adcomm

I put my name, LSAC number and "Personal Statement for X Law School" (or Addendum, or Optional Essay, etc.) at the top, and page numbers (1 of 2) at the bottom.
