Header for PS

Is a header or title expected for a personal statement?

It only takes up room. The only thing I did was label the title "Personal Statement." And I labeled each of my 2pgs with a header in the top right with my full name and page #. I only did this in case the adcomms would be printing my material. I did not want them to lose pages.

I had (on one line): my name (left justified), Personal Statement (centered), LSAC # (right justified)

and then "Page x of y" in the footer

In my header I had left justified " Name - LXXX" with the page number in the right corner of the header.

also, I used this format on ALL my attachments, resume, PS, disciplinary addendum.
