Re: book title - italicize, quotes or underline?
From my experience in undergrad, italics. However, there have been instances (prior to undergrad), where I have used both quotes and underlining when referencing a title of a book.

Italics for books, quotes for scholarly articles, underlining for third-graders.

Quotes are for short works. Both italics and underline are technically correct for titles of books. Italics wasn't always standard because it didn't work on typewriters and early italics in fonts were not always readable. The important thing is consistency.
