Do I need to fill out a tax form?

I'm currently in UG, but during 2010, I made $1000 as a TA and I received a $4500 scholarship. I'm confused on what to do because of my FAFSA. Do I need to fill one out and use that when filling out my FAFSA? I'm totally ignorant about this stuff, so please don't spare me any details. Thanks!
